2010-2011 Grade Level & School Confirmation Form

List the players with School Official’s signature (Principal or Teacher) for all players on your roster. The Varsity Coach or AD can also confirm players. This is needed for all grade levels. Players in the 6^th grade cannot be 13 years old prior to 6/1/10. 5^th grade players cannot be 12 prior to 6/1/10. 4^th grade players cannot be 11 prior to 6/1/10. 3^rd grade players cannot be 10 prior to 6/1/10.
